Medication Taken At School
If your child must take medication (either prescription or over the counter) on the first day of school, the Medication Permit Form must accompany the labeled medication. This form requires a physician’s signature even when the medication is over the counter. These forms are available on our website: www.sjecs.org.
Medication must be in the original container, with the child’s full name, type of medication, dosage and physician’s name. The school cannot administer medications without the appropriate form(s). Antibiotics may not be administered by school personnel. (Archdiocesan Policy 5300)
